Efficient USCIS Compliance for Immigration Medical Exams
An immigration medical exam is a key step for employees seeking an adjustment of status to become lawful permanent residents. Rather than a standard physical, this screening ensures that all applicants meet USCIS-defined health and vaccination requirements.

What Does an Immigration Medical Exam Consist Of?
For an immigration medical exam, the applicant will be assigned to one of MOH’s U.S. Civil Surgeons, medical doctors officially designated by USCIS to perform medical examinations for lawful immigration. Supported by a dedicated team of medical assistants, our clinics are fully equipped to manage the entire exam process from start to finish.
Medical History and Physical Evaluation
Our U.S. Civil Surgeon reviews your employee’s complete medical history, including any past illnesses or hospitalizations. A physical examination is then conducted to assess for any physical or mental conditions that may require follow-up.
Required Laboratory Testing
Per USCIS and Centers for Disease Control and Prevention (CDC) guidelines, specific tests are mandatory for applicants based on age:
- Drug abuse screening: This evaluation identifies any history or presence of substance-related health issues.
- Tests for syphilis and gonorrhea: These tests are required for all applicants age 15 and older. We can help arrange treatment for anyone who tests positive.
- Tuberculosis (TB) testing: We use advanced blood tests (QuantiFERON® or T-Spot®). USCIS no longer allows the use of a TB skin test.
Vaccination Verification
Applicants must show proof of immunizations. If your applicants need required vaccines, our team can provide them on-site. Common requirements include MMR, Tdap, polio and hepatitis B, depending on age and the current CDC schedule.
Communication and Interpreter Requirements
Please note that MOH does not provide or coordinate interpreters. If your employee requires language assistance, an interpreter must be present for the duration of the in-person office visit. At the conclusion of the exam, the interpreter must physically sign the official paperwork. Digital signatures are not accepted for this process. In accordance with I-693 requirements, computer-generated or artificial intelligence (AI) interpretation tools are not permitted.
How Long Does an Immigration Medical Exam Take?
While the in-office physical and blood draw typically take about an hour, the entire process—including lab processing and the finalization of the I-693 form—usually takes five to seven business days.
Official Verification
Once all medical results are returned and verified, our Civil Surgeons will sign the official I-693 form. Your employee will receive the original document in a sealed envelope. The envelope must remain sealed until opened by a USCIS official. If you or your employee breaks the seal, the document will be rejected.
MOH provides a separate, unsealed copy of the results for your employee’s personal records and your HR files.
